Community Health Centers of Lane County (CHCLC) uses a multidisciplinary team approach to provide medical, mental health, and substance abuse treatment to patients at their four clinic sites. Homeless patients are served at all sites.
Each patient has a designated medical team that includes Access Specialists who screen and help patients connect with programs offered by the CHCLC as well as other community partners that help homeless patients meet their basic needs.
Services include assistance with Medicaid applications, referrals for shelter, transitional housing, education, employment, and SSI/SSDI application assistance.
Community Health Centers of Lane County has a prenatal clinic, family planning services, pediatrics, family medicine, internal medicine and behavioral health services. Homeless and at-risk youth receiving services at a nearby day access center are referred to the Charnelton Community Clinic and offered transportation to the clinic.
