Subacute Treatment offers brief and intensive on-site psychiatric services that stabilize a young person after a significant emotional or behavioral incident with the goal of achieving behavioral and mental health stabilization in order to prepare each child for the next phase of care. Subacute services provide more intensive care than what is offered through Trillium’s Psychiatric Residential Treatment Services.
Eligibility:
We serve children and adolescents, ages 5 – 17, who are in need of 24-hour residential treatment for challenges related to mental health. Subacute services provide more intensive care than what is offered through Trillium’s Psychiatric Residential Treatment Services.
Intake Instructions:
Clients with OHP are often referred through their County Family Care Coordinator, Wrap-Around Coordinator, or other OHP representative.
See website for a list of information needed to make a referral: https://www.trilliumfamily.org/referral-information
