Wraparound is a team-based planning process that supports youth and families with complex needs. Wraparound brings youth and families together with a team of friends, relatives, schoolteachers, counselors and caseworkers to develop an individualized plan that is based on their vision of success.
Eligibility:
Youth under the age of 18, must have multiple-system involvement (mental health, juvenile justice, special education, DHS, physical health complexity, developmental disabilities, etc.), connected to their coordinated care organization through the Oregon Health Plan.
Intake Instructions:
Anyone connected to a youth, or the youth themselves, may submit a referral for Wraparound. The referral packet is submitted to the Wraparound Program Supervisor. From there the youth is scheduled for the Wraparound Review Committee, where it is determined if Wraparound is the best fit for this youth and family. The referral process is not immediate, and can take up to a month for determination.
