Pathways to Adulthood Independent Living Program is a Department of Human Services- (DHS) funded program designed to provide life skills in nine specific areas for foster youth, 16–21 years old, to transition out of foster care. This program is a referral-based program a DHS case worker must submit a referral from within Multnomah, Washington, or Clackamas Counties. Transition services include subsidies for the older youth who can meet requirements.

Eligibility:
Native American foster youth ages 16–21 years old. You must have a referral from a Department Human Services caseworker in Multnomah, Washington, or Clackamas Counties.
